I think it is Smokey that cannot leave the island. MIB is dead and Smokey took on his likeness. Just like Locke is dead (we have seen his body) and now Smokey has taken on Locke's likeness. Smokey still can't leave the island. If MIB would have lived long enough to get the donkey wheel...
