Here's one instance, I'm sure there are more. Employee of US military contractor admits faking strength-test results for steel in Navy submarines, a decades-long fraud

A metallurgist has pleaded guilty to decades of fraud after she admitted faking the results of strength tests of steel used to make US Navy submarines.

Over the 30 years, Thomas falsified results for more than 240 batches of steel, a "substantial percentage" of what the firm supplied to the Navy, the statement said.

When confronted with the doctored results, Thomas told investigators, “Yeah, that looks bad,” the Justice Department said. She suggested that in some cases she changed the tests to passing grades because she thought it was “stupid” that the Navy required the tests to be conducted at negative-100 degrees Fahrenheit

Seems like she didn't agree that the testing requirements were necessary but instead of doing the right thing and getting them changed she just ignored them.  Big no-no!

It's pretty amazing to me that she faked reuslts of steel testing results for so many years ------ steel for submarines. Seems like there should've been more checks built in to the system. They're not cafeteria trays.

This was why people #####ed and complained about cell phones and airplanes.  Extensive studies were being conducted with rigorous standards to be as close to 100% certainty that there was no interference.  The fact she did this astonishing in a bad way

A lot of standards have gray areas that are open for interpretation, and a lot give outs that a manufacturer can have a rationale for deeming parts of a standard non-applicable or using other means than those recommended in demonstrating compliance.  But what this woman did was outright crazy and fraudulent.

I really don't understand how it was possible for one person to fake strength test results.  Almost every job our company has done for the government has had multiple witnesses present when doing anything similar to that.  

How could such an important test be unwitnessed?

There definitely should have been other checks. My dad was head of quality control for a company that supplied parts to the US government and had to fire a guy when he caught him doing the exact same thing. Kind of scary since my dad had to personally sign off on every shipment that went out. If he hadn’t caught it, he could have been liable if something happened.

I can’t imagine the guilt that woman would have had if something failed and a submarine went down.
