I was just wondering how and when the pre-season schedules are determined. Do they work like college non-conference schedules, where teams negotiate games? Does the league set them up? TIA for sharing the info.

It's really just convenience for most teams.Teams like to play non-conference teams that are reasonably local.
Philly plays Pittsburgh and the Jets every year, and they've played Baltimore since they came to Maryland.
The league usually sets the 4th (or 5th) game for television's sake, and if the teams aren't tapped for TV then they take the 4th based on convenience.
Some are "rivalries" - like Jets / Giants - that happen each year.
Geography often dictates, and the teams like to drive rather than fly often.
It did strike me as curious as to why the Cardinals would fly cross-country twice in the same pre-season, since they have games against both NE and Pittsburgh.
