I added this in my leagues this year, figured I’d share the form I created for it. Works for 6 team playoffs. 

We are doing $25/bracket. Winner takes all.

Standard bracket rules. Call each round of teams, 1st/2nd place byes, etc.  Catch is, you fill it out BEFORE the season starts.

To clarify since two people asked: 

You are filling this out with your FFL team names, basically attempting to predict your fantasy league Champ bracket. 

It adds some fun #### talking / rooting wrinkles during the 2nd half of the season and playoffs.
