We have gates that don't allow people to follow you through. The only way would be if I were to scan my card twice, once for them and once for me. And I take great delight in NEVER doing so.
We had proximity card lock set nice and low to be handicapped accessible. Lock didn't always open on the first try. With my wallet in my front pocket, sometimes I had to hump that thing.
We have these gates with the two glass saloon doors. You scan your badge then input a PIN# and the doors open like on StarTrek then close behind you (shiish, shiish). If someone tries to jump in behind you they get sliced up like that cow on Under the Dome.
Every office I've worked at with keycards always had a Plan B. You forget your keycard, you go and get a temporary one. You also get heckled by the guy/gal giving them out.
