I’ve been off meds for gout for years. It’s possible to rectify through diet alone.

ETA: and exercise. A lot of it.
I exercise every day in some way and my diet has no meat and only occasional seafood, so it’s going to be hard to improve it that much. Cherries and vitamin C seem like the main things I could increase. Maybe skim milk, though I try to stay away from dairy.
Do you drink coffee? I didn't and started. Coffee helps clear either purine or uric acid. Everyone is different, becoming a regular coffee drinker helped me.

Interesting. I have not seen that come up in anything I had read before... wouldn't really matter since I hate the taste of coffee but yea, I do not recall seeing anything on that.
 
Gout is one of many reasons I dislike very low carbohydrate diets, which tend to lean heavily on animal products. That said, the two things most strongly linked to gout are alcohol and fructose intake.

And yes, coffee consumption reduces the risk of gout - unclear if it’s due to lowering uric acid, or anti inflammatory properties.

Purine is in some veggies as well. What I found is that everyone is different. And is affected by different foods/amounts.

Soda is poison. Do you drink booze?

Milk thistle is a thing. Turmeric also. Any and all anti inflammatories are good

I've never really had a sweet tooth, but I finally recognized that an occasional ice cream binge would trigger gout attacks in the worst way. I would chalk it up to high abv beer, naturally, not realizing the sugar was what was driving the worst of it.

I mention this in case it's a possible blind spot you might have.

Don’t really eat sweets and don’t drink any soda. All I drink is Zevia. Is that a problem?

I do drink beer, though have been pretty light on that for the past 2-3 months actually. It might be a trigger, but it must be very haphazard because ive gone through various periods drinking a fair amount of of high abv beer and still only had an occasional flareups.

Will be looking to add more cherries, vitamin C and turmeric. And whenever I can get a PCP and get in there to figure out possible medication.

Also continuing with weight loss.

Beer is the alcoholic beverage most strongly linked to gout. Have you tried abstaining from alcohol altogether, for at least a month or two?
 
It is not possible to significantly drop high uric acid levels to normal range without medication. I tried everything, every supplement, every diet. Finally went on Allopurinol and have seen a significant reduction in URIC acid levels.

Gout is truly different for everyone. What works for one, will not work for another. Same for triggers

Agree with triggers, don’t agree with significant, long term reduction of high URIC acid by anything other than meds.

My triggers are mushrooms, cauliflower, and cured meat. The first two are so bizarre to me, lol.
 
I'm still going to tout that staying hydrating and drinking significant amounts of water consistently is a top preventative measure.

Agree with triggers, don’t agree with significant, long term reduction of high URIC acid by anything other than meds.

I must be an outlier then. Allopurinol did nothing for me. I still had attacks on it. Uloric worked much better. My goal was to not be only daily meds. I figured out how to do that. A lot of trial and error with diet. hydration, less booze and a ton of exercise haven’t taken meds for 10+? Years

You are correct for the masses that won’t change what they’re doing and just want a easy fix.
 
Been on Allopurinol for a while now and it worked like a charm... until a few days ago. A small flair up. Not too bad but I am on Eliquis so I can not take any NSAIDs. Reached out to my Dr and she prescribed Methylpredisone and that did the trick immediately. I am walking around like nothing happened right now and honestly it has me pissed off that my other Dr didn't prescribe this when I had major flair ups before. He prescribed something else that only marginally helped and kept me in pain for a long time before it went away.

How much water do you drink on average?

How much water do you drink on average?
I try to make it a priority. When I am on it (like now) around 100+ ounces a day. If I am not, which is to say, I go through my day focused on other things and before I know it the day is over, guessing around 60ish. Those are rookie numbers and I gave myself a goal of 120 a day as min.
 
May has been a brutal month. First I had 2 left wrist flare ups, unbelievable pain, was prescribed steroids, which helped, but doc said I must see a hand specialist, as he thinks I may have gout. My calls were both tele health with no bloodwork.

Then I just got hit with unbearable kidney stone pain. Seeing a urologist tomorrow. I’m careful to be my own google doctor, but I’ve been around enough misdiagnoses to peek at chat gpt for info. 85% of stones are calcium, but 15% are uric acid created. So now I’m left wondering, is my entire blood just filled with crystallized uric acid, causing both gout and kidney stones?

I 100% acknowledge I am obese, don’t eat well, don’t drink enough water, take meds for HBP and cholesterol, drink too much, and don’t exercise enough. I will make changes. But I’m going to push for Allipurinol. My buddy has been on it for 8 years with no flare ups. He also did all the lifestyle changes too and feels better, lost weight. I’ve clearly beaten up my body for too long and I must stop.

Bloodwork isn’t how gout is diagnosed. While elevated blood uric acid can suggest the possibility, technically, you’re supposed to sample joint fluid.

But aspirating a joint is an invasive procedure, one many doctors don’t feel comfortable performing. Even if they do, it takes time, so bloodwork is used as a shortcut.

The problem is, up to half (maybe more) of acute gout attacks occur in those with normal serum uric acid levels.

Still, just playing the odds, you certainly sound at risk. I’d definitely come up with a concrete plan for lifestyle modification moving forward. Even if you don’t have gout, all the changes you alluded to will likely improve your quality, and quantity of life.

Yea... gout hurts a lot until you have kidney stones and then you wish you had gout again instead all over your body. 100% high uric acid contributes to kidney stones. Calcium stones and obviously Uric stones.

The biggest thing you can do to keep the gout and kidney stones is hydration and change diet. The best hack I have found is Crystal Light. This helps you drink more water (at least for me it does) and it has a lot of citrate in it. Citrate does two things, 1) prevents bonding of calcium stones and 2) alkalizes the blood, lowering uric acid levels. I am pretty sure that my most recent flair up after years of not having any is due to me getting away from drinking the Crystal Light. So, I am back to drinking it as my most common beverage if not drinking just water. Allpurinol helps but don't get into the trap of just taking the pill and thinking you are good.

Pro tips: I get Costco water and then Aldi version of Crystal Light. Easy and very cheap. I also will add flavors with the Mio type flavors from Aldi or whatever else to the Lemonade to not just drink Lemonade all the time.
