When it's called. Perreria has said on many occasions that the play stops when the ref determines it's stopped, not when the ref's reflexes get the whistle to his lips and blow.
 
The solution is for a bad TO to be either a delay or unsportsmanlike conduct foul, including having to keep the same personnel on the field unless they follow it up with a real TO.

But bottom line is that the solution is ABSOLUTELY NOT to have the ref look away from the play to see who is calling the TO
Agree completely but the solution is to keep things the way they are. Lets look at the root of this issue- how does having a coordinator calling time outs affect the game? The answer is it doesnt, this is a parsing of the rule book PURELY being done by fans unhappy with the outcome of the game. Its a total technicality. The head of referees has made it clear that they are doing the only responsible thing- watch the field and if you hear a time out from the sideline, blow the whistle. Anyone on the sideline is deemed eligible, reasoning that by rule all players and the HC are eligible, and by extension any of the coaches have been granted the HCs proxy to call a timeout.

This is entirely rational. Again- organizationally if a coordinator calls a TO he surely has the authority of the HC to do so. If not that is a interteam problem the HC has to deal with. So how does it affect the play of the game who calls the TO as a representative of the team?

Any nonsense about referees hearing fans or NFL employees fooling them is a ref hering. No different than somebody in the stands (or a camera man?) blowing a whistle etc. If somebody in close proximity to the ref asks for a TO, he calls it. If it happens to be Pam Oliver, we deal with it as we would any 3rd party outside interference in the game.

I cant believe how much nonsense has been discussed based on the refs giving the Ravens what they were trying to do.

 
Ref heard TO called prolly thgought it was the HC and blew the whistle. Noone should have a issue with that. That was Ryans fault just as much as the refs.

What Ravens fans can legit be upset with was the 4th down D Holding call ( the second one on the drive - the first one on 4th was legit as Moss was mugged) . That IMO is the only call that really can questioned.
