So we moved into a new house that has a perfect spot above the fireplace for a TV. Cable jack and power outlet already installed.

Question - how do I mount the TV up there and avoid having boxes/wires/etc all over the place? Currently my TV is on a stand on an entertainment center that contains all of that stuff.

 
Get a Wall Mount and a shelf for the cable box, etc. If there is an outlet and cable jack up on the wall (to be hidden by TV), you should be all set. Did something similar in my bedroom, except I didn't have the jack/outlet up on the wall, so I had to go behind the sheetrock. NBD.

The shelf has a space to hide wires, looks real nice. I have a cable box and an AppleTV on mine, bunch of cables, but they are mostly hidden. And from a distance, you don't see them, if they are all black.

Rip out some drywall and run the wires to wherever you want. If you just moved in and are re-painting, it's even less work as you'd be doing the re-painting anyway. The worst part of ripping out sheetrock is trying to match the paint if you're not re-doing the whole wall.

 
Drill small holes in the wall behind each component to be connected (TV, STB, speakers, etc.) Pass the wires through to the room behind the wall and then back through to the component to be connected.

If someone points out the ugly wires in the other room(s), throw them out of your house.
