A friend of mine, openly gay lives with his significant other, has a birthday coming up. I usually don't buy birthday gifts for adults but he's making a big deal about his birthday coming so why not.

Plus he's always the one that goes overboard when I have a get together at my house (most people just bring a bottle of wine, he'll show up with a huge sushi platter or something extravagant).

He's totally cool; he handles and even initiates gay stereotype jokes all the time.

I'm trying to come up with a gag gift but something that he'll use/keep.

Seriously, he won't be into it sexually, but I think if he's cool making jokes and whatnot on the regular that he'd get a kick out of it. The stripper would definitely be into it, which is to everyone else's enjoyment also. Certainly checks the box on over the top gift, too.

I have a hard enough time trying to figure out what to get my wife for her birthday. You expect me to know what to get a gay man?

 
My wife has several customers that are gay middle aged men so we are buying gifts regularly:

If he likes to cook a nice knife is always a good gift (gay or not)

Nice bottle of Grey Goose or Ciroq is always a good last minute gift

My wife and I have started giving the gift of experiencing something to all of our friends rather than buying stuff (unless there is something in particular we know that they would like)

Spa Day at a nice spa

Dinner at a nice restaurant

Ziplining tour

Recently bought a dinner and flight around the skyline of dallas on Living Social for a good friend and his partner - they loved it - said it was the best gif he got.

Pop your girlfriend's buttplug in, drop your pants in front of him at the cake cutting ceremony, announce to the crowd "A GIFT FROM ME TO YOU!!!" and then bend over and spread your cheeks.
