Both of Bautista's homers were on pitches out of the strike zone. And yes, Madson was not even close to Perez's location on Bautista's second dinger.

Not sure. Moustakas did work to a 3-0 count so in the driver's seat to break it open. Looks like he had 4 sac bunts this year, but not sure about how effective he is at it.
