I would guess it is event driven, meaning the phone call triggers the recording instead of just continuous recording.  But I would bet different locations do it differently.

It depends on the recording system they are using. If they are using the trunk side recording system, what you say before they answer will be recorded.

Most systems record even while the phone is ringing.  I watch ID too much and even the prosecutors didn't know that.
