What are the cut off points for the number of entries in a pool? Small GPP sounds like it would be between 0-500. But, I could be wrong

Not sure if there's an exact answer to this. I can ask some of the guys who designed the app if you want a more specific answer, but I would personally think of small as anything under 100. You probably want a stack with most quarterbacks, but you don't necessarily need to think too much about having any risky plays or having multiple low ownership guys.

Medium in the 100-500 range. Definitely want at least one unique guy and I'd start thinking about fading some of the highest owned players.

And large as 500+. There's a real art to these huge GPPs. Much more likely to double-stack if I'm using one of the more expensive QBs. And try not to have more than 2-3 of the really highly owned guys.

Yes, please do that, as this question has been asked and left unanswered several times in the Crusher thread. Options like this should be clearly defined within the app or the instructions for the app.
