My ex works at a hospital admitting patients.  They just had mandatory training on how to deal with every person they screen.  They heave to ask what their biological sex is as well as what they indentify as.  And they cannot use he or she or Mr. or Mrs.  The always have to address people by their first name.  She's had some interesting reactions that's for sure.  Some people have said they were insulted by getting asked what sex they identify as.
