I just checked thesaurus.com for "stain".

All 3 of those words listed are on the primary list of synonyms for stain. Any of those 3 should be the correct answer.

I kind of agree with you, but it's interesting that "tarnish" is sitting at 100% right now. If everybody agrees that that's the best synonym, then that sort of makes it so.

It's the 1st word listed. So, I'd argue that if you changed the order of the words, you might see a different response. Also, I'd guess that "tarnish" is probably used more often in regular conversation by most of us over both blotch and sully. But, that doesn't mean that those other words don't essentially mean the exact same thing as stain.

Blotch because it is a synonym as both verb and noun.

I think question four is all synonyms too, but infirmity is specifically weakness while the other two imply the loss of strength.

 
1 and 4 are pretty bogus here. Argument could be made for any of the answers. Guessed wrong on 3.
