Anyone had anything similar?

Lower abdominal pain that stretches around 360 to my side and back. The pain is dull, not always present, comes and goes. Occasional testicle pain. Sometimes a slight burning feeling when urinating.

I don’t know if I should see a urologist or a nephrologist, or if maybe it’s my appendix or gall bladder. Is there a one stop test my primary care doctor can order to screen that whole area?

 
Not a doctor, but I'd recommend seeing one asap.  Just go to primary care doctor and let them tell you what kind of specialist you need if you're unsure.

I'd go so see a regular doctor. Don't wait on stuff like that.

Either way, you should get seen soonish depending on severity of your symptoms. You don't need a specialist to start. Try to get into your primary care doctor for a sick visit. If you can't get in soon for whatever reason, then an urgent care or even an ER visit would be order.

Thanks everyone. I will call my primary care on Monday. I called one day last week but couldn't connect (staffing issues). I think this is what I have - checks off almost all of the boxes. My google searches hit on a lot of things but not this one. I appreciate all the advice and will report back soon. 

 
The good aspects of Urgent Care in your case, if it becomes more painful: Cheaper than the ER, faster than primary care, some tests available immediately such as a urine test to rule out a urinary tract infection. You'll get a professional diagnosis of likely issues, treatment, and recommendations. Two months ago,  I had severe urination frequency for 2 days, with minimal pain, so I went to urgent care on Sunday afternoon. It was quick. Test for UTI was negative, the PA said it was likely an enlarged prostate. $75. On Monday, I called my urologist's office, and there were no appointments until the next week. 10 minutes later, I passed a small stone and the urgency went away. If you have kidney stones, the advice is to drink lots of fruits.  (Not a doctor or nurse).

 
Honestly--I have no idea what you could have--nor am I qualified to guess. With that said-I echo the sentiments of others that say that you shouldn't wait around with what you are describing.  You should definitely see and consult with a doctor. Hopefully it's nothing serious, and I wish you the best

 I had that. It was a pretty unpleasant experience.

 
I have benign cysts on my epididymis that cause some of those symptoms including nut pain, burning sensation during urination, and occasional abdominal pain. 

I was given the option of having them lanced/removed or just deal with the occasional discomfort. So far I've gone with the latter.

You guys are pretty good. My doctor had 3 off the top of his head guesses. First inclination is prostatitis, He ordered blood and urine labs, which I did, and am waiting for results. He also mentioned appendicitis but didn’t think so because the pain would have been more localized to the one side. He also said maybe kidney stones but then quickly sad he didn’t think that (he didn’t explain why he was quick to dismiss, I wish I had asked him why). Hoping for a call soon about the labs. I feel good today with no symptoms. Had mild symptoms  yesterday while in his office. Very strange - will keep you all posted - thanks all!

Keep us posted. I don't know about you and kidney stones for I had them last year. Doubled over with sharp pain in back right quadrent, pain was intense.  ER visit with 2 morphine shots to stop pain.  Ended up having to get shock wave surgery few weeks later for stones were big 8 and 11mm. Kidney stones are a one of a kind pain you'll know. 

 
The way it's coming and going seems to me to suggest stones. Movement to the wrong place equals pain while elsewhere not so noticeable.

My doc called and left me a VM saying that all of the blood and urine labs came back normal. One thing he was looking for was elevated PSA levels which would have suggested something amiss with the prostate. Good news that nothing showed there, but I don't know if that's conclusive. I set up a teledoc call with him later this afternoon. He mentioned on the VM to discuss next steps - he said either a GI doctor (and getting pre-auth for an abdomen cat scan) or a urologist (and getting pre-auth for an ultrasound of the family jewels). I think he honestly is not sure which direction to go in and may leave it up to me? If I can choose I would think the bigger priority and bang for the buck would be the cat scan, right? it show a larger area and bigger array of potential problem spots? That is where I am leaning at the moment. Symptoms are still mild to non-existent the past few days, which is good.

Honestly - I would see both (depending on insurance and co-pays and such).

This is when you nip issues in the bud before they become bigger issues. Now that's I'm 52 I tend to take this stuff a lot more seriously, especially when you might be getting an early warning sign.

Others had a lot of ideas early on, but I haven't seen hernia mentioned.  Does it hurt worse when trying to lift or push/pull something?  My only other thought is gallstones, if the pain is on the left side.

My wife had undiagnosed gall stones for months. Pain would come on at night and be terrible. Finally took a trip to the ER to get a proper diagnosis after a scan showed she was loaded up with stones. Took the gall bladder outpatient laproscopically and no more issues.

Edit to add the gall bladder is on the right side and not the left.

LOL.  I've had my gall bladder out and I was still wrong.  I have a different scar on the left that I sometimes remember as my gall bladder, but the gall bladder was laparoscopic and I often forget about the three little scars.

If symptoms are getting better/resolving, then it's not unreasonable to wait and see how things progress. If it flares back up, then get seen at the time you are having symptoms. 

It's not as big of an issue with adults at it is with kids, but ultrasound is without radiation. That said, ultrasound is better for looking at certain things and CT is better at others. So, it shouldn't be left up to you. The doctor ordering the test should have an idea of what he/she is looking for and then order the appropriate test to evaluate that. If they aren't sure, then go see one of the specialists they recommended and they can figure out what you might need. With it involving the nether regions, I would suspect a urologist is probably going to help get to the answer a little better.

i had the teledoc call and the doctor recommended both tests (abdomen scan and nether ultrasound)and to then see both specialists (GI and urologist). With regards to the GI he also mentioned getting screened for colon cancer and getting a colonoscopy. He said I'm basically at the age (48) where I should be doing that anyway. So I think he is just trying to check off all possible boxes. He said it is good news that the blood/urine labs came back clear but that it of course means we don't have any extra clues as to what's going on. The test pre-authorizations may take a bit but I'll post updates when new info is known. Thanks all!

 
Did you have a urinalysis?  With Kidney stones, there will most likely be blood in the urine.  Just because you can’t see blood in the urine does not mean that it isn’t there.  Stones can also lead to Urinary Tract infection.  

 
Some updates.
 

The ultrasound on the family jewels came back fully clear.

The CT abdomen primary finding was 2 non obstructing kidney stones, 1 at 5mm, the other at 4mm. Secondary finding was Colonic diverticulosis without evidence of acute diverticulitis Third finding was fatty liver disease, which I already knew about from 3 years ago. 
 

Got into the GI doctor first. They recommended a fiber supplement to help the diverticulosis from becoming diverticulitis. Also doing a fibro scan to make sure no liver scarring. They want me to go on Mediterranean diet to lose weight and trim the fatty liver. Not supposed to be drinking either. Crap. Urologist visit is in 2 weeks.

Other CT findings (listed but not considered major) - arthritis, enlarged prostate (will ask urologist about), mild plaque build up in arteries, small bilateral fat containing inguinal hernias - there is also a small fat containing umbilical hernia (PC and GI both think no big deal, but said I could see a surgeon). 

I guess when you’re 48 and don’t consistently exercise and/or follow a healthy diet, this is what you get.

Latest symptoms are increasing lower back pain (I just fell down the stairs while sober, and landed on my butt and lower back, that ain’t gonna help), occasional front abdominal pain (very light), frequent urination, slight burning while urinating, mild ball pain. Probably mostly the kidney stones, I guess.

@NajehHejan I have diverticulosis.  Some recommendations in order of perceived benefit (most of this is just good for overall  gut health in general):

fiber (recommend citrucel)

lots of water

eat dinner early

i have some other gastro issues and take a probiotic and digestive enzyme also

Thanks so much for the tips. Just the past week I started drinking 64-96 oz per day - will make sure I stay on top of it. Have not bought my fiber yet, but will look for citrucel. Dinner early is interesting- may be tough with schedule but will try. May I ask what your symptoms were that made you find out you had diverticulosis?

My stomach overall has given me problems over the past few years.  Sometimes not that bad, sometimes worse.  Saw a gastro dr who checked me out and said I had IBS (which is actually code for stomach issues they can’t figure out…science!) and also diverticulosis.

I had a diverticulitis attack last October and now that I’ve read up on it and went through all the work with the Dr. It’s easy to identify…basically very localized pain in lower left abdomen area.  Usually sticks around a few weeks and if it’s bad or not abating I’d recommend getting antibiotics.  For some people it can be every painful.  
 

So I’ve been tinkering with different things, the heart of my strategy is to support digestion as much as possible and have everything cycle in and out of my intestines smoothly and quickly lol.  So far so good for the past 5 months.

In terms of not eating late yah that’s hard.  If I eat late, particularly meat that’s where I’ll take the digestive enzymes to help with digestion.

Btw, for folks with IBS I’d recommend checking out IBGard, expensive but I’ve found it very helpful when I’m in a flare.
