Holding a phone on your shoulder to your ear (1 Viewer)

I've never done this. I can see in the old day when the phones were much bigger, like hard wired ones or the first cordless. But how do you hold a flat smartphone like that?

I saw a guy yesterday with nothing in either hand walking down the block doing it. Is it that much trouble to hold your phone to your ear?
