I know of a couple in Manhattan that both have it and they've only left their apartment twice since March 2 to go to the grocery store. They did everything they were supposed to: keep social distance, don't touch face, use hand sanitizer, wash hands thoroughly when you get home. But they now have it.

Same for one of our pastors. Did everything right - all that you listed plus wiped down packages, pulled the kids out of school when we had a handful of cases, started social distancing weeks earlier than anyone I know (wife/mom has underlying conditions.)

Still got it. Everyone recovered without hospitalization, kids only had low grade fevers, adults got their butts kicked for a solid week.

For protecting others (outward protection), using the same reference value of 100 particles produced by coughing a home made mask lets out 90, a surgical mask 50 and an N95 respirator 30.

From what I read, the virus is really tough to get off of polyester. Seeing mostly polyester or fleece gaiters/buffs/balaclavas. Not sure they’re actually better than nothing as you may increase your own chances of getting exposed.

The studies linked in this article seem like homemade masks are way less effective.

I think it’s likely that they’re better than nothing assuming that people are using them properly 95% of the time.

From my experience seeing people with any types of masks in public so far, I’d say that over 50% have not handled them properly. I’ve seen loads of people having them hanging from their face to breath easier (what’s the point of even having it), touching the mask with bare hands, touching their face, moving the mask aside to eat or drink, etc.

I think the idea behind it is good, but with how few people are using them properly, I don’t think we should expect them to have a very large impact on number of infected. Worst case is that the early fears of the CDC (my assumption) end up being correct and it actually increases transmission as people become less vigilant about other better methods of reducing transmission ie. people starting going out more instead of staying home , touch their face more, don’t wash hands as often, etc.

Jared Kushner made it clear this week that they are treating the national stockpile as belonging to the federal government rather than as a resource for states to tap into.

 The notion of the federal stockpile was it's supposed to be our stockpile," he said. "It's not supposed to be states' stockpiles that they then use."

The next day, the website for that agency was changed:

That’s why NY took it upon themselves to source ventilators from a foreign government.

i've seen some chatter about how perhaps the federal stockpile was sold to China as part of their reponse to the outbreak. the belief that, despite the advice of experts, there was an opportunity to make some money by the administration and others. China, for its part, needed enough PPE and equipment but they quickly went through their own reserve and were looking. now, if this true, it does explain why the federal response has been so uneven.

I would call the hospital and see what they say. If they are really desperate they may take it. There are reports of some hc folks who are put in a situation of using homemade masks which doesn't protect hc workers from contracting it.

The issue is that public beaches make up a fraction of total beach property, so when they open them, everyone goes to the same spot, a recipe for disaster. Which is why public beaches need to stay closed, imo

Close off the large parking lots and big accesses to the beach, increase the law enforcement patrols, etc. 

It's very doable.

Not every beach looks like those spring break photos.

I'm saving mine for more something more dire than getting groceries. Like if I have to care for a family member or neighbor that gets sick. I'm not even going to wear a mask to the store, it's simple enough to stay away from people.

I think they have because the states preferred option #2.  There is simply not enough time to get so many beds and ventilators to save all the patients.

I think many states would have preferred a cohesive public health strategy offered up by the Feds but from the start instead of the White House saying there wasn't any worry. A coordinated public health strategy - where states could evaluate there needs in real time - for States to put orders in for equipment et al and identify shortfalls, where the Feds could step in, is a sensible strategy. it's not how i would have gone about it but it's a viable on its face. Instead we have a free for all and the White House telling states to "sorrynotsorry".

Good post. The knowledge base is constantly evolving, and the available data must be interpreted with caution.

AFIK, we don't even know the infectious dose of SARS-CoV-2. Even finding complete viruses in the environment doesn't mean they are readily transmissible from surfaces in sufficient quantities to cause infection. Moreover, the idea that it can be spread by things like speaking and breathing is speculative at best.

To use everyone's favorite comparison, influenza is predominantly droplet spread. This means larger droplets which travel shorter distances (~6 feet or less) cause most infections. Hence, droplet isolation with surgical masks is recommended. But there is evidence that flu, like SARS-C0V-2, can be spread in aerosols in some situations. And the original SARS appears to have some aerosol spread, though run-of-the-mill coronaviruses are thought to be droplet.

A combination of epidemiological, environmental and experimental (usually animal) data facilitate understanding the likely mechanisms underlying infectious spread. ATM, we're still learning about this virus, so any dogmatic proclamation should be viewed with skepticism. Here is a nice review of some issues with determining aerosol spread.

IMO, things which favor some aerosol spread include: 1. similarity to SARS-CoV-1 2. the tendency to cause lower respiratory tract infection 3. higher Ro than seasonal flu - but there are many other potential reasons for this.

That being said, it certainly isn't as contagious as something like measles, the classic example of an airborne illness. 

My gestalt is it's mostly droplet spread + some fomite, with a smattering of aerosol, fecal-oral and other mechanisms. For the average person with average exposure risk, social distancing + good cough and hand hygiene will probably be sufficient to limit spread. Regarding universal masks, it's hard to know if they help by limiting droplets/aerosols from infected individuals, decrease the size of innoculum for uninfected people who are exposed, and/or prevent people from touching their faces. It's probably some combination of all three.

Good posting. Since I'm taking care of patients that are all either positive or rule-out COVID-19, been wearing a full PAPR ensemble. We are opening a whole hot-zone wing for COVID, that will be full airborne precautions. Should be up and running by the time I go back on Tuesday.

On the bright side, I've probably lost 15 lbs. sweating in all that gear for 12 hours at a stretch. Reminds me of high school wrestling practice.

1) immediate term:  enforce near total nationwide shut down for three weeks, test millions a day, isolate and contact trace new positives; coordinate a federal response re: ventilators, PPE; use country's wealth to keep consumers and businesses solvent.  Reopen.

2) short-term:  ramp up antibody testing so that future wave social distancing can be more measured, with previously positives staying at work

3) short-to-medium term:  share learnings from major hospitals and ramp up research to find treatments that reduce cascading failures/fatalities among gravely ill.

4) longer-term:  develop vaccines and, in very long term, possibly antivirals.

I feel like this is a good plan. In which case, and I hope it doesn't happen, please watch a video on the proper way to "don and doff" (put it on and take it off). It would be a real shame if someone had the means to protect themselves and inadvertently contaminated themselves just taking off the mask.

Exactly. At Trader Joe's where limited numbers are allowed in and 6 ft rule, there is no way you are 6 ft from the cashier. And the dude I had was sniffling. I stood there trapped praying he wouldn't sneeze. He was very chatty too.
