It was reported on ESPN Radio. No link yet that I can find. They speculated he would be out for a few weeks.

 
If there were any life in this offense, I'd say upgrade D-Jax due to his past chemistry with Dilfer. But I don't think we can even say that.

As much as I would love to have Norv back, I think there is a bigger problem than the OC. Oline is horrible. How the hell do you let the fatest SOB on the def to walk right up the middle and smash your QB? PATHETIC!!!!!
 
Nolan was asked if Smith will play next week vs the ravens, his response, "probably not".
We do not know anything about anything until it is confirmed this afternoon, ESPN is simply going off a SF Chronicle article which stated that there were "unconfirmed reports" of 4-6 weeks.That is no report, that is a guess.I will post any pertinent info to the blogger after Nolan's presser.

ESPN.com reports San Francisco 49ers QB Alex D. Smith (shoulder) suffered either a second or third degree shoulder separation during the team's Week 4 game, according to the San Francisco Chronicle. A third degree shoulder separation would likely require surgery which would most likely end Smith's season.

 
Maybe a change in the QB is just what the Niners need right now. They have 3 of the worst 6 offensive performances of any team in the league this year. I have a feeling Nolan was already thinking of giving Dilfer a shot, but the decision is easy now.

As for Dilfer, I don't think he played that badly, the line didn't give him much time. I also think it's obvious that they miss Vernon Davis, because he was a great blocker and opposing teams had to alway put a man on him. It's no surprise that Peterson had a good game, normally he'd be covering Vernon.

Dilfer just didn't look prepared out there. I think with a whole week of preparation he'll play much better next week. Too bad it's versus the Ravens.

I'd like to see Shaun get a shot, he's always looked good in pre-season. But, I don't think Nolan should put him in unless the season was lost or Dilfer was completely worthless.I have to say that the San Francisco fans have been way too forgiving towards Smith. If this was another city he would have been driven out of town with pitch forks a long time ago. He hasn't shown anything in his 3 years to prove that he was worth the first overall pick. If he is indeed lost for the season, the Niners should just consider him a bust and search for a new starting QB in 2008.

It doesn't matter who the QB is, they will play badly behind this line. I would rather have Smith in there instead of Dilfer because Smith is more mobile.

I agree the Oline looks bad. I don't understand what's changed since last year, because they were a good crew last year. I think maybe Larry Allen is finally showing his age, he got beat a couple of times. Something on the line has to change though, (Baas over Smiley and maybe Snyder over Allen).

Lets not forget that he is only 23 years old and still has time to mature. I think hes the youngest starting QB in the NFL. But I somewhat agree with you. He hasnt shown much improvement since hes been there, but hasnt he had 3 different OCs in the last 3 years? Maybe he will come around, but Im not so sure he will. I do think its time to look elsewhere at the end of the season unless he shows some major improvements. The injury wont help his cause.

DJax hasn't done anything with Smith in there, so it can hardly get any worse. Perhaps the aforementioned chemistry from their Seattle days may be a slight benefit.

Good news for me. Ravens are really starting to suck but with this matchup coming there is HOPE :shock:

he did pretty good in the afc title game and was two dropped passes away fromn a super bowl mvp.
 
This might end up being a non-factor for a lot of Smith owners. You wouldn't play him this week or next week, and his next nice match-up is in week 8 against the Saints. Will he be healthy by then? I don't know, but if so, he won't miss a start for me or lots of people using QBBC.

"Just a grade 3 separation"? That's a complete tear of the shoulder ligaments. Muir Orthopedic

The treatment of Grade 3 AC separations is somewhat controversial. Many studies show no difference in the outcomes for surgically treated separations versus doing nothing. A significant portion of people who undergo surgery will need another operation later as the injury causes the joint to degenerate and become painful. Other physicians feel that some patients benefit from surgical repair. One case where repairing the ligaments may be best is in the case of the highly functioning throwing athlete. Some would argue that these athletes perform better following repair than without the repair.

In some Grade 3 shoulder separations, surgery may be suggested. Surgery involves relocating the joint and repairing the torn ligaments. A screw, or some other type of fixation may be used to hold the joint together while the ligaments heal. This surgery is done through a 4-5 inch incision over the acromioclavicular (AC) joint. If a screw is used to hold the clavicle in place while the ligaments heal, it will usually be removed six or eight weeks after the surgery. If not removed, the screw will probably break.

Grade III separations are more serious. Some surgeons advocate surgery to stabilize the AC joint. Others feel that immobilization in a sling for 3 weeks, followed by intensive physical therapy give just as good result. In any case, it is important that the shoulder be painless and have full range of motion and strength before the athlete returns to sports.
This is a big deal (if you believe Smith is important) that has been downplayed so far. Realistically, surgery or no, you're looking at a minimum of 4 weeks out and probably more like 6-8 weeks depending on progress, and significant atrophy in the shoulder during that time which will cut down on his arm strength when he returns.

Brian Griese had a grade-3 separation in 2000; he missed five games, threw 2 passes in the sixth, and didn't play in their playoff loss. That was his best, Pro Bowl season, where he had 19 TDs and 4 INTs in 10 games before the injury; he was never the same afterwards.

I think you should get used to Trent Dilfer at QB.

 
Alex Smith lost for season. MRI showed that he torn 2 tendons that connected his Collar bone and his shoulder tore.

the part about the two tendons i saw on NBC 11 and the sports reporter said that he tore the two tendons.

Its not a definite confirmation. Its still bad news.

Your post read like it it was a done deal. Which isnt quite the case.

Though some could assume (as I do) that he is done for the year.

A team source has told ESPN's Ed Werder that San Francisco 49ers starting quarterback Alex Smith is seeking a second opinion from Dr. James Andrews after preliminary MRI results indicated he appears to have suffered a serious injury to tendons in his right shoulder in Sunday's game.

On Monday, Smith was cautiously optimistic.

"They wouldn't put an exact date on it, but yeah, a few weeks," Smith said when asked to estimate his return. "Having the bye week helps, but I'll just push it as hard as I can and try to get back as soon as possible."

Smith had taken every snap for the 49ers over the past two seasons until Rocky Bernard, the Seahawks' 308-pound defensive tackle, landed squarely on him during a sack. The Niners lost 23-3 to fall to 2-2.

The former No. 1 overall draft pick struggled along with the San Francisco offense so far this season, going 43-of-84 for 461 yards with one touchdown and a meager 66.4 passer rating.

On Monday, Smith said he completely tore both of the ligaments that connect his shoulder blade to his collarbone. He knew he was seriously hurt as soon as he hit the ground underneath Bernard.

"I've taken lots and lots of sacks like that, and if you fall a fraction of a way different, you get up and you're fine," Smith said Monday.

With pain shooting through his body, he attempted one practice throw on the sideline during Monday's practice before heading to the locker room.

"It didn't go very far," he said with a grimace. "I'd like to think I have a little bit of pain tolerance, but I knew something was wrong there."

With Smith out, the 49ers will turn over their struggling offense to veteran Trent Dilfer, who went 12-of-33 for 128 yards Sunday in his first game action since 2005. He'll get his first start for the 49ers against the Ravens on Sunday, the team Dilfer led to its only Super Bowl victory.

Dilfer has started 107 games for four NFL teams during his 14-year career, but didn't play a snap last season or in the first three games this year. He has focused on being a mentor to Smith and a help to the offensive coaching staff while still staying ready to play.

"Not a whole lot changes except the repetition in practice," the 35-year-old Dilfer said on Monday. "I get to work on timing like we did back in training camp, and I think that will help our execution. We're obviously going to have to execute better on Sunday."

49ers want second opinion on ESPN report

Don't panic about that ESPN report regarding Alex Smith's shoulder. Smith is not seeking a second opinion from noted orthopedic surgeon James Andrews at Stanford as ESPN reported last night. Instead, the team's head trainer, Jeff Ferguson, is friendly with Andrews and has sent Andrews a copy of Smith's MRI results, which were taken Monday. It's standard procedure at this level.

I thought the ESPN piece seemed funny when it mentioned the serious tendon damage Smith had suffered. Instead, Smith suffered torn ligaments, which are expected to scar over and heal on their own. Smith certainlly suffered a serious injury and his collarbone is detached from his shoulder. But the collarbone is essentially in the right spot -- if it wasn't, there would be a need for surgery -- and Smith is expected to make a full recovery.

Mike Nolan today on the ESPN report that Alex Smith is seeking a second opinion from James Andrews and might have season-ending surgery: "Anything you've heard about Alex is incorrect. You don't need to waste your breath on it." Nolan did say that a physical examination of Smith's right shoulder would be conducted later today by team trainers. If anything changes regarding the QB's diagnosis and prognosis, you can find the facts here. For alternative theories, projections, guesswork, etc. click onto ESPN.

Dr. James Andrews, the noted orthopedic surgeon in Birmingham, Ala., today confirmed the team's original diagnosis on Alex Smith -- he has a Grade 3 shoulder separation and does not need surgery. The team sent Andrews the results from Monday's MRI.

Niners quarterback Alex Smith is still saying he'd like to return to action Oct. 21 against the New York Giants after sustaining a separated right shoulder on Sunday against the Seahawks.

"That would be the goal," Smith said. "The bye week helps."

The swelling is going down in his right shoulder and the pain is subsiding, he said. He hopes next week he can begin throwing a tennis ball and then build up to throwing a football.

"I'd like to push it next week," Smith said.

The 49ers play the Ravens on Sunday. Trent Dilfer will get the start at quarterback. Next week, the 49ers are scheduled to practice Monday through Thursday before taking the weekend off because of the bye. When the 49ers come back, they will begin preparations for the Giants.

Smith said he does not plan to undergo surgery -- now or after the season. He spoke with Dr. James Andrews this morning. Andrews told him the everything looked good. Smith is still wearing a sling, but he hopes to be able to start moving it around, which will expedite the recovery process.

Smith said the ligaments were torn are not vital to the process of throwing. The ligaments generally scar over. As Smith spoke with reporters, there did not appear to be any obvious deformity in his shoulder.

"It's a matter of getting that strength back," Smith said. "The collarbone wasn't displaced much at all. The dislocation wasn't significant at all. Everything around it is pretty healthy and kept it intact. It's the movement that's going to help it, get the pain out, and you build up pretty quick."

Smith said he was told the torn ligaments in his shoulder do not factor in his ability to throw a football, adding, "It has nothing to do with the throwing motion. As far as playing with it, there's no danger of hurting it ... except from landing on it."

Smith continued to wear a sling on Thursday, although he does not sleep with it. He said treatment at this point consists of ice and electrical stimulation. The smile on his face indicated he's pleased with the progress he's making.

"It definitely feels significantly less than it did two days ago," he said of his pain level. "It's tender. I definitely have more movement that I did two days ago. The swelling is pretty centrally located. It's gotten progressively better every day.

"I hope a couple weeks from now I'll be able to practice. I'd like to practice next week. Who knows?"

The 49ers are scheduled to practice Tuesday, Wednesday and Thursday of their bye week.

While Smith said he is taking pain medication, he said Andrews did not think a cortisone injection to the shoulder is necessary.

The athlete in Smith wants to get back as soon as possible. He said watching Trent Dilfer, who will start Sunday against Baltimore, run the offense during practice made him realize he wanted to fast-track his return. The 49ers will opt for caution.

"Trent running what was your huddle, you do want to get back as fast as possible," Smith said. "That's the biggest goal. They're (the 49ers) going to err and be cautious and safe. I want to get back for my team. The bye week (following the Baltimore game) helps."
