2015 crash in Czech Republic. 3 died on the train, another score injured on the train, and by debris that flew into a train station up the tracks. Truck driver had just stopped there, then pulled forward just far enough to save his cab. He could have drove through the barrier, it is made so even a motorcycle can dislodge it, and doing so would have automatically set the brakes on any train on the line. The train driver saw the truck and was only able to slow from 99 to 88 mph by the time of impact.
