You can also ask the casino for copy.My wife won $5k on a slot machine last year and now i cant find the form they gave us. Do i need it or any of the info on it? I know the exact amt she won.
Don't they ask for name and tax ID number of the casino. I am sure you can call the casino and get it. The IRS can give you the information on the forms file against you , but thatight take a few weeks to get. They just send that info to the IRS. As long as you know the amount, you're good to e-file. Just fill out the electronic W2-G.
no taxes were withheldIf they withheld any tax, I think you may need that form. At least to get any credit in a timely fashion, if at all.
Then as long as you report the gross amount as other income, you should be fine.no taxes were withheldIf they withheld any tax, I think you may need that form. At least to get any credit in a timely fashion, if at all.

Unlikely that will be adequate documentation for the IRS. They want you to keep a log with casino, time of day, number on machine, total won/lost, etc. For every single gambling session.if you have a players account you can log in and print your history for the yearNobody keeps receipts of this.Did she also lose money at gambling during he year?
