My wife won $5k on a slot machine last year and now i cant find the form they gave us. Do i need it or any of the info on it? I know the exact amt she won.

They just send that info to the IRS. As long as you know the amount, you're good to e-file. Just fill out the electronic W2-G.
Don't they ask for name and tax ID number of the casino. I am sure you can call the casino and get it. The IRS can give you the information on the forms file against you , but thatight take a few weeks to get.

If they withheld any tax, I think you may need that form. At least to get any credit in a timely fashion, if at all.

AC. And it was my wife which is essentially losing
Did she also lose money at gambling during he year? Cause you usually can offset them.
Nobody keeps receipts of this.
if you have a players account you can log in and print your history for the year
Unlikely that will be adequate documentation for the IRS. They want you to keep a log with casino, time of day, number on machine, total won/lost, etc. For every single gambling session.

Unless you're audited, they won't bat an eye. $5000 is hardly an amount that triggers review.

That is a log. I use it every year.

It has my dates, buy ins, winnings, etc.
