I don't know if it's been used in previous cups, but I'm going to go out on a limb and say teams started using the lying down player about 10 seconds after a player kicked one under a jumping wall.

I think it started when they changed the rule and the opposing team had to be a yard from the wall. Not sure why it correlates but that's what I remember.

I know nothing about refs in soccer at all, but I recall the announcers saying at the beginning of this match that this particular ref has a habit of "escalating situations." Quite prescient.
