My Dad tested positive today with a BinaxNow test. He's going for an official test tomorrow at the hospital, his doctor doesn't seem to want to do anything until an official test.

He's vaccinated (no booster), 71, severely overweight, has high blood pressure, and not sure what other comorbidities.

He's 700 miles away from me, but from what I  understand from phone reports from my mom, he has what I'd describe as medium+ symptoms, which started 2 days after exposure (it's now 4 days after exposure).

Exposure was at his Church. 12 others have tested positive so far.

My mom has no symptoms (yet), has not taken a test.

My Mom (and another family member from my wife's side) actually mentioned Ivermectin and (my mom) joked they could get it from my sister (vet tech). I want to punch someone.

For those of you with more experience from this side, other than "listen to your doctor", should I be pushing them to do anything? Earlier treatment is better, right? Should he ask for monoclonal antibodies or an antiviral?

Best of luck. My 74 year old, 40 BMI, mobility challenged father is just finishing with mild symptoms. No booster either. The vaxx is a game changer.

So excited for my five year olds vaxx tomorrow. Too bad she tested positive today.

Ugh, sorry to hear that, but I hope it's a normal young case and she recovers splendidly. My biggest fear has been getting to this point for my 10 year old before all the infection risks caught up. We still have a week to go for his second shot.

Given your dad's age and conditions -- and how early in the infection he is -- yes, definitely ask for the monoclonal antibodies if the doctor hasn't suggested them already. Your dad is just the kind of patient most helped by monoclonal antibodies.

And yes, earlier treatment with monoclonal antibodies is absolutely better.

My pop received monoclonal antibodies, but they are thin on the ground around me. Should be trying multiple avenues to get on waiting lists ASAP.

Gyms around here reduced capacity, which makes more sense for limiting infection, but less for business costs.

He won’t qualify for antivirals (Only remdesivir at this point, as the newer drugs haven’t received EUA yet) unless his oxygen saturations are low. But he is a candidate for the mAbs (Regeneron, etc.), which he should get ASAP.

He shouldn’t take dexamethasone, or traditional antibiotics, unless compelling evidence of a secondary bacterial infection. 

And I wouldn’t recommend ivermectin. Same goes for D, Zinc, C and whatever other supplement du jour YouTube is promoting - while those three won’t hurt, they probably won’t help either.

Tylenol for fever. And cough suppressants (dextromethorphan, Tessalon) are probably better than expectorants (guaifenessin).

Thank you, this is very helpful. I passed this along and hopefully gave him the right encouragement this morning to talk to his doctor immediately.

Follow up question about your thinking on cough suppressants vs. expectorants. They've already received family advice to take Mucinex DM and I'm just passing along info from a Doctor I know on the internet, so I need some additional info about this if possible. ;)

He did report he's feeling better this morning than he has the last two days and I guess he was able to use his c-pap machine last night. He said he is only taking Nyquil and ibuprofen (for some kind of leg pain) so far.

I'm not a doctor, but with what you mentioned about his weight and HBP I'd be looking very hard at monoclonal antibodies like 48 hours ago but ASAP now. 

 
Well, FDA just approved boosters for all adults.  Now CDC needs to do so, recommend that ALL get it, and state unequivocally, that you are NOT considered fully vaccinated 6months after shot #2 (or 2 months after JJ) unless and until you get an additional pfizer/moderna shot.  Today.  Now.

I am scheduled for my booster but it was very difficult finding a place that would do it because I fall outside of the CDC guidelines

Did he get this test (bolded above)? What I would consider doing is check to see where he can get the monoclonal antibody infusion, and see if they can also issue a test first at the facility (tell them he tested positive on a Binax at home test). Medicare will cover the cost of the treatment whether he has Medicare Advantage or Original Medicare (it is typically covered under Part B). It's not that Medicare will require the test, but the practice where he will receive the mAB infusion will have their process, which from what I've seen typically does include a positive PCR test or antigen test.

While Germany is encouraging a third shot, 2G protocols consider anyone two weeks outside their second shot as vaxxed.

I dropped in at our county building yesterday morning to get my booster (original JnJ end of May) and there was a line out the building.  Took me about an hour, no post jab observation window and got my app updated to reflect my new status.

Son is having a friend stay overnight tonight.  Kid's family are anti-vaxxers and then we find out the mom is 'sick'.

He got the test this morning, they're supposed to have results in 24 hours. 

I'm going to try to research a facility near him, thanks.

 
Quick question for you guys, that I'm having trouble finding the answer for. I had a proven case of Covid19  

I had my immunity tested and my IGg came back at 11.44   I know that >1 shows immunity, but how high does the scale go?  

 
So far, Covid is working out for my kid. Just got back from Costco with a TV for her room and an iPad so she can facetime friends and family.

She's getting sick as a 5 year old and coming out a teenager.

I found the following list of US facilities that provide mAB treatment and sent it to my parents. They called around and found the locations near them are mostly hospitals part of the same network and they need to wait for the test results that they took this morning (at the same hospital network).

So now we wait. My sister dropped off some OTC meds at lunch, so hopefully he'll be more comfortable. And at least he's actively participating in calling around and is taking this more seriously than he did last night.

In case this helps someone else (list of mAB treatment centers):

Thanks for posting the list. Hopefully the fact the PCR test facility and the mAB provider are part of the same system will help facilitate things if needed. Rooting for a quick and easy recovery for your Dad.

It’s not critical, but expectorants cause some people to go into coughing fits, which can be problematic if oxygen saturations are marginal. If he’s tolerating the guaifenessin, no big wup.

In general, I’d minimize NSAIDS like ibuprofen in older people - added risk from kidney and gastrointestinal side effects as compared to Tylenol. There also was some concern they’d lead to worse covid outcomes early in the pandemic, though good data doesn’t really back up that sentiment.

Also realize CPAPs can aerosolize SARS-CoV-2, so make sure he and the machine are well isolated from vulnerable people.

And I don’t think Medicare requires a positive PCR, but many healthcare providers don’t fully trust antigen test results.

That’s been a point of contention throughout the pandemic, and I don’t believe anyone knows the correct balance. FWIW, my thick skull usually favors controlling the pandemic, as I don’t think we’ve fully mastered not overwhelming healthcare systems. It’s easier to recover from recession than death.

With a couple more antiviral options on the horizon, I’ll probably be more in line with your thinking in a couple months.

Depends on the test, and there’s no universally approved interpretation of levels beyond positive and negative.

I got my second moderna shot in April and then got covid in September.   under your plan I would now be unvaccinated.  my doctor recommended that I hold off on a booster for now.

Agreed; I'm no anti-vaxxer by any means, but how long are we supposed to keep up this damn song and dance?  

I finished getting Pfizer jabbed in May and am going to wait until end of December / early January to get the booster.  

But certainly not looking forward to having to do this every 6 months until .... ???

My plan?  A third shot is plainly required 6 months after the second shot.  Seems like in your case, you could have used it a little sooner.  A breakthrough infection would be a different circumstance -- the issue is that the CDC is so piss poor in communication, that giving this nuance will result in neglect.

Test came back positive, sounds like he's now waiting for a call from the treatment center to set up an appointment for the mAB infusion. That all changed fast. :thumbup:

We all do know at this point that getting the vax you have a .001 of hospitalization.  Unvaxx (this doesn't include natural immunity) is .89 percent.
